• For Exercise or Nutrition Services

For fully bulk billed sessions, please ask your GP to fill out one or more of these referral forms:

​

• For all chronic injuries/diseases - Up to 5 sessions

• If you are also of Aboriginal and Torres Strait Islander background - Up to 5 additional sessions

• If you also have Type 2 Diabetes - Up to 8 additional sessions

For motor vehicle accident injury claims, the following forms need to be submitted to the insurance company:
